     The undersigned claimant hereby releases all laborer and materialmen lien rights against the above described project on the basis stated as below.  This release is for the benefit of and may be relied upon by the owner, the prime contractor and the construction lender.

     This release is CONDITIONAL and shall be effective only upon receipt of payment by the undersigned in the sum of $__________________ .

     This release is PARTIAL  and effectively releases all lien rights for labor performed and materials delivered to said project up to and including the date of ______________, beyond which date the claimant reserves all rights to file a Notice of Claim of Lien for labor performed and material furnished to the project after said date, if payment is not received for work performed after said date.

     If payment is by check, this release is effective only when the check is paid by the bank upon which it is drawn.
